Overview

FAMILY CONNECTS CHICAGO @ MOUNT SINAI HOSPITAL

Sinai Chicago-Mount Sinai Hospital is a one of the hospitals selected to participate in Family Connects Chicago, a pilot program through the Chicago Department of Public Health.  Family Connect is an in-home nurse service available to Chicago families with newborns. The service is simple: A registered nurse connects with you around 3 weeks after birth to check on the health of the recently delivered parent, baby and the whole family. Typically the nurse team would conduct a home visit. However, due to the COVID-19 pandemic, families are offered the option of a virtual visit if they are not comfortable receiving in home visitors. There is no additional cost to you, and anyone with a newborn can participate, including foster and adoptive parents.

Everyone deserves extra attention and encouragement when a newborn arrives. Family Connects Chicago nurses provide a comprehensive spectrum of in-home support, guidance and connections to resources—at no additional cost to you—including:
